How to Fill Out Form-1095-A

  • Your 1095-A includes information about Marketplace plans that any household members purchased in 2021. Before you file, double-check that everything is correct. If it isn’t, contact your Marketplace call center and hold off on filing until you receive a corrected version.
  • Cannot find your Tax Form 1095-A? Check your state’s agency website online.
  • Already submitted an incorrect version? In that case, submit an amended return to the IRS.
