If your household members had Marketplace coverage in 2021, you’d use Form 1095-A (Marketplace Health Insurance Statement) to complete the 8962 Form, Premium Tax Credit.
How to Fill Out Form-1095-A
- Your 1095-A includes information about Marketplace plans that any household members purchased in 2021. Before you file, double-check that everything is correct. If it isn’t, contact your Marketplace call center and hold off on filing until you receive a corrected version.
- Cannot find your Tax Form 1095-A? Check your state’s agency website online.
- Already submitted an incorrect version? In that case, submit an amended return to the IRS.
