Medicare Supplement Plans Near San Francisco, CA: 2026 Guide

San Francisco residents approaching age 65 or already on Medicare often face a confusing decision: how to cover the gaps left by Original Medicare. Without proper coverage, a single hospital stay or specialist visit can lead to thousands of dollars in out-of-pocket costs. Medicare Supplement plans, also called Medigap, are designed to fill these gaps by paying for coinsurance, copayments, and deductibles. For seniors in the Bay Area, finding the right plan is essential to managing both healthcare access and monthly budgets. This guide covers how these plans work, what options are available, and how to choose a policy that fits your needs in San Francisco.

How Medicare Supplement Plans Work

Medicare Supplement plans are private insurance policies that work alongside Original Medicare (Part A and Part B). When you receive medical care, Medicare pays its share first, and then your Medigap policy pays some or all of the remaining costs. This structure helps protect you from unexpected medical bills, which can be especially high in a city like San Francisco where healthcare costs are above the national average.

There are ten standardized Medigap plans labeled A through N. Each plan offers a different level of coverage, but the benefits for a given plan letter are the same across all insurance companies. For example, Plan G from one insurer covers the same core benefits as Plan G from another. The main difference is the monthly premium, which can vary significantly between carriers. When shopping for Medicare supplement plans near San Francisco, CA, it pays to compare prices from multiple providers to find the best value for your situation.

It is important to note that Medigap policies only work with Original Medicare. If you are enrolled in a Medicare Advantage plan (Part C), you cannot use a Medigap policy to supplement it. Some people switch from Medicare Advantage back to Original Medicare during the annual enrollment period, and then purchase a Medigap plan. However, doing so may involve medical underwriting, which can affect your eligibility and premium.

Why San Francisco Seniors Need Medigap

San Francisco has a high concentration of top-tier hospitals, including UCSF Medical Center, Zuckerberg San Francisco General Hospital, and California Pacific Medical Center. While access to excellent care is a benefit, the cost of that care can be substantial. Original Medicare covers 80 percent of approved charges after deductibles, leaving you responsible for the remaining 20 percent. For a surgery or extended hospital stay, that 20 percent can amount to tens of thousands of dollars.

A Medigap policy eliminates or drastically reduces this financial exposure. For example, Plan G covers the Part A deductible, Part B coinsurance, and excess charges. This means you pay only the monthly premium and the Part B deductible (which is around $226 in 2026). After that, nearly all covered services cost you nothing out of pocket. For seniors on fixed incomes, this predictability is invaluable. It allows you to budget for healthcare without fear of surprise bills.

Additionally, San Francisco has a diverse population of retirees, including many who moved to the city later in life. If you are new to the area, understanding local healthcare networks and insurance options can be overwhelming. A Medigap plan offers the freedom to see any doctor or specialist that accepts Medicare, which is the vast majority of providers in the city. There are no network restrictions, no prior authorizations for specialist visits, and no need to choose a primary care physician.

Comparing Medigap Plans: Which One Is Best

Choosing the right plan depends on your health needs, budget, and risk tolerance. Below is a comparison of the most popular plans for San Francisco residents.

• Plan G: Covers everything except the Part B deductible. It is the most comprehensive plan commonly purchased today. Premiums are higher than Plan N, but out-of-pocket costs are nearly zero after the deductible.
• Plan N: Covers most costs except the Part B deductible and Part B excess charges. It also requires a small copayment of up to $20 for doctor visits and $50 for emergency room visits. Premiums are lower than Plan G, making it a good choice for healthy seniors who want to save monthly.
• Plan F: Covers all gaps, including the Part B deductible. However, Plan F is only available to those who became eligible for Medicare before January 1, 2020. If you are new to Medicare, you cannot enroll in Plan F.
• High-Deductible Plan G: A lower-premium version of Plan G with a deductible of around $2,800 in 2026. Once you meet the deductible, the plan pays 100 percent of covered costs. This option works well for people who want a safety net but prefer to pay less each month.

For most San Francisco seniors, Plan G offers the best balance of comprehensive coverage and predictable costs. It eliminates coinsurance and excess charges, which are common in expensive medical markets. If you are healthy and want to save on premiums, Plan N is a strong alternative. Just be aware that Part B excess charges can occur if your doctor does not accept Medicare assignment. In San Francisco, most doctors accept assignment, but it is worth confirming with your provider.

When to Enroll: The Medigap Open Enrollment Period

The best time to buy a Medigap policy is during your six-month Medigap Open Enrollment Period. This window begins on the first day of the month you are both 65 or older and enrolled in Medicare Part B. During this period, insurance companies cannot deny you coverage or charge you more due to pre-existing conditions. This is known as guaranteed issue rights.

If you miss this window, you can still apply for a Medigap policy, but you will likely face medical underwriting. Insurers can review your health history and may charge higher premiums or deny coverage based on conditions like diabetes, heart disease, or recent surgeries. For this reason, it is critical to enroll on time. Many San Francisco residents delay enrollment because they are still working or have coverage through a spouse. However, once you lose that coverage, you may have limited time to enroll without underwriting.

There are also special circumstances that grant guaranteed issue rights outside the open enrollment period. For example, if you lose your Medicare Advantage plan due to the plan leaving the area, or if you move out of the plan’s service area, you may qualify for a Medigap guarantee issue period. If you are considering switching plans, it is wise to consult with a licensed agent who understands California’s specific rules.

Costs of Medigap Plans in San Francisco

Premiums for Medigap plans vary by carrier, your age, and sometimes your location within California. In San Francisco, monthly premiums for Plan G typically range from $130 to $250 per month, depending on the insurer and whether the policy is community-rated or issue-age-rated. California is one of the few states that uses attained-age rating for many Medigap policies. This means your premium increases as you get older, rather than being based on your age at enrollment.

Some insurers offer community-rated policies, where everyone in a given area pays the same premium regardless of age. These can be more affordable in the long run, especially if you buy a policy at age 65 and keep it for decades. When comparing plans, ask each carrier about their rate structure and historical premium increases. A lower initial premium may lead to steeper increases later, while a slightly higher premium from a community-rated plan might offer more stability.

To get an accurate picture of costs, request quotes from at least three to five insurers. You can use online comparison tools like those on InsuranceShopping.com to see side-by-side pricing for Medicare supplement plans near San Francisco, CA. Be sure to enter your exact zip code, as rates can vary by neighborhood. For example, premiums in the Marina district may differ slightly from those in the Sunset district, depending on the insurer’s rating area.

How to Choose an Insurance Provider

Selecting the right insurance company is just as important as choosing the right plan. In the Medigap market, financial stability and customer service matter because you will likely keep the policy for many years. Look for insurers with strong ratings from A.M. Best (A or higher) and low complaint ratios from the California Department of Insurance. Companies like Mutual of Omaha, AARP/UnitedHealthcare, and Anthem Blue Cross have a strong presence in California and offer competitive plans.

Another factor to consider is the insurer’s history of rate increases. Some companies raise premiums aggressively, while others keep increases modest. You can request rate history from the insurer or check with the California Department of Insurance for public filings. A company that has raised premiums by only 2-3 percent annually is generally more predictable than one with double-digit increases.

Customer service is also critical. If you have a question about a claim or need help understanding your benefits, you want a company that answers quickly and clearly. Read online reviews from San Francisco policyholders to get a sense of each insurer’s reputation. You can also ask friends or family members who have Medigap coverage for their recommendations. Frequently Asked Questions

Can I buy a Medigap plan if I have a pre-existing condition?

Yes, but only during your Medigap Open Enrollment Period or a guaranteed issue period. Outside those windows, insurers can deny coverage or charge higher premiums based on your health history. It is best to enroll when you first become eligible.

Does Medigap cover prescription drugs?

No. Medigap plans do not cover prescription drugs. You need to enroll in a separate Medicare Part D plan for drug coverage. Some Medigap plans sold before 2006 included drug coverage, but those are no longer available.

Can I switch Medigap plans later?

Yes, but you may face medical underwriting. If you want to switch from Plan N to Plan G, for example, you will need to apply and may be denied or charged more if you have health issues. Some states have special rules that make switching easier, but California does not have a birthday rule or annual guaranteed switch period.

Are Medigap premiums tax-deductible?

Medical expenses, including Medigap premiums, may be tax-deductible if you itemize deductions and your total medical expenses exceed 7.5 percent of your adjusted gross income. Consult a tax professional for your specific situation.

How do I find plans that cover my San Francisco doctors?

Since Medigap plans allow you to see any Medicare-approved provider, you do not need to worry about networks. Just confirm that your doctor accepts Medicare assignment. Most San Francisco doctors do, but you can call their office to verify before enrolling.
